Appointment Requests

Our appointment request form lets you ask us to schedule an imaging service. Submitting a request is not a confirmed appointment. An appointment is only confirmed once AMI contacts you to verify the date, time, and details. We will make reasonable efforts to respond to requests within one business day, but we do not guarantee availability of any specific date or time.

SMS / Text Message Consent

By providing your phone number on our appointment request form — or verbally when you call us — you consent to receive text (SMS) messages from Advanced Medical Imaging related to your scheduling and appointment communications. Consent to receive text messages is not a condition of receiving any imaging service.
